Welcome to ENT Specialists, Inc.! Here is everything you need to know before your first visit.
- Please arrive 15 minutes early to complete registration
- Bring a valid photo ID and all of your insurance cards
- Bring a list of current medications and dosages
- Bring any relevant imaging (CT/MRI/X-ray on disc) and reports
- Bring your referral if your plan requires one
- Write down your questions and symptoms in advance
Insurance Information
At ENT Specialists, we participate with most major insurance carriers — HMO, PPO, and Indemnity plans.
